3. Executive Affiliate #XXXXXXXX [(because if they are not in SFI they don't care or know about bronze, silver, gold or platinum even means)and the number just encase they need it]
4. Day and night phone # (unless you don't want them to call you)
5. Web address/Domain name [(I would suggest you purchase a domain name so that it is a simple address without affiliate numbers, some will delete the numbers and enter as no-ones affiliate) assign your your web address to go to your www.sfi1.biz/XXXXXXXX.XX]
Note: I would use this affiliate link because when they arrive, they see your name welcoming them. Use a key code so that you know what web address they arrived from. You can have more than one domain name going to the same affiliate link.
Remember to keep it simple! When I sold print advertising I remember people wanting to put 500 words in a two inch ad, it can be done.... but the probability of anyone reading it is very slim.
I would also leave the back blank... so they can take notes when they visit your site.
